Health Outreach Worker

You will deliver a holistic Health and Wellbeing Programme to meet the needs of Young People whose lives are affected by their own, or a family member’s, substance use. You will support Young People who are homeless or at risk of homelessness, or having difficulties living with family members. You will work in partnership with Hillcrest Futures to co-ordinate, plan, develop and deliver this programme.
You must demonstrate an understanding of the issues faced by Young People affected by substance use, and have experience of working with young people in a range of settings.
You should have a degree or equivalent in Nursing, Community Learning and Development or other related discipline.
