Health Improvement Officer
37.5 hours per week
Fixed-term- 14 months or secondment
Band 5 (£26,104 – £32,915)

This post holder of Health Improvement Officer will deliver Young Carer Health Checks to meet the needs of Young Carers in school, college and school leavers. This role will include working to develop and deliver evidence based Health Checks to Young Carers and working in partnership to address identified unmet need, in a manner that promotes their autonomy and independence.
You must demonstrate an understanding of the issues faced by Young Carers and have experience of working with young people in a range of settings. You should have a degree or equivalent in Nursing, Community Learning and Development or other related discipline.
Health Outreach Worker
37.5 hours per week
Fixed-term – 14 months or secondment
Band 6 (£33,072 – £40,736)
Closing date: Wednesday 8th December 2021
You will deliver Young Carer Health Checks to meet the needs of Young Carers in school, school leavers and College. You will co-ordinate, plan, develop and deliver evidence based Health Check’s to Young Carers, and work in partnership to address identified unmet need, in a manner that promotes their autonomy and independence.
